How can I make a Lasting Power of Attorney (LPA)?

Access the OPG Online (OPGO) Portal: Log in using your Singpass to complete the LPA form. Go to OPGO →
Appoint donee(s): Specify one or more trusted individuals to make decisions on your behalf if you lose mental capacity.
Certification: Book an appointment with our doctors to certify your LPA. Our doctor will ensure you understand the LPA and are not under duress.
Submission: The LPA certificate will be submitted online via the OPGO portal.

How do I make an Advance Medical Directive (AMD)?

Download the AMD form from the Ministry of Health. Visit MOH's AMD page →
Complete the form in the presence of our doctor by visiting our clinic. Our doctor will confirm that you understand the AMD and are of sound mind.
Submission: Our clinic will submit the completed AMD form directly to the Registrar of AMDs at the Ministry of Health on your behalf.

What is the difference between LPA and AMD?

An LPA lets you appoint someone to make welfare, property and financial decisions for you if you lose mental capacity. It's broad in scope and only takes effect once capacity is lost. An AMD is narrower: it's a direct instruction to doctors not to prolong life with extraordinary treatment if you become terminally ill and unconscious, with death imminent. Many people set up both as complementary parts of their planning.

Can I make both an LPA and an AMD?

Yes. They serve different purposes and most people who plan ahead choose to set up both, often together with an Advance Care Plan (ACP).

Do I need a lawyer to certify my LPA?

No. An LPA can be certified by an accredited doctor, such as ours, instead of a lawyer, provided there are no complex circumstances requiring legal advice.
